Currently, novel coronavirus vaccinations are being administered diligently at healthcare facilities throughout the city.
Through these vaccinations, we aim to reduce illnesses and severe cases of the virus, quickly bring an end to infections, and expect to bring socio-economic activities back to normal.
To all business owners, we ask for your attention in creating an environment in which your employees can easily access vaccinations, including allowing employees to be exempted from work duties in order to be vaccinated, in accordance with business conditions.
Novel Coronavirus Vaccination Guidance
Vaccination tickets have been sent to all eligible residents in Takikawa City, and going forward reservation periods will be open to all ticket-holders.
Vaccinations are not compulsory and will only be given with the patient’s consent. The currently approved vaccine is deemed to be 95% effective with 2 doses and prevents symptoms including fever and cough.
We ask that you please read the enclosed materials carefully and consider getting vaccinated in order to reduce the number of infections and deaths from the coronavirus and lessen the burden on the healthcare system.
We will also provide vouchers to those at least 65 years old or those who have disabilities in order to help offset the costs of transportation to/from the vaccination site. Please see the following page for details (Japanese info only).
1.Process for receiving your vaccination
Takikawa City has learned that vaccine supplies from the national government are being reduced. Therefore, the city is taking reservations for the number of residents for whom it has secured enough supply to administer the two required doses.Vaccinations will continue until all residents who wish to be vaccinated have been, but we recommend getting vaccinated early. Depending on how reservations progress, the number of healthcare facilities and the length of reservation periods may be reduced in the future.
When you receive your vaccination ticket and registration opens for your priority group, you will be able to reserve an appointment at a designated vaccination site.
2. Vaccination ticket (Takikawa City Novel Coronavirus Vaccination Ticket)
Vaccination tickets are required when making a reservation and at your vaccination appointment. Please be careful with it.
Tickets are in the form of stickers. Please bring the whole backing sheet to your vaccination site without removing the sticker(s).
You will also need to bring a form of ID (e.g. My Number Card, driver’s license, health insurance card, residence card) and your medical questionnaire.
※A special form for senior citizens will be used in some cases.
3. Vaccination sites
The healthcare facilities listed below are conducting vaccinations.
In order to make a reservation, please follow the procedure outlined below. Please refrain from contacting the healthcare facilities directly.
4. Medical questionnaire (for each of the 2 appointments)
The vaccinations are given in two doses, and a medical questionnaire is required for each appointment.
Please read the questionnaire carefully, fill out the information in the bold-outlined box in advance, and bring the completed questionnaire to your vaccination appointment.
Your second appointment will simultaneously be scheduled for approximately 3 weeks after your first appointment.
5. Informational paper regarding the novel coronavirus vaccine (manufactured by Pfizer)
The vaccines being given at this time are those manufactured by Pfizer-BioNTech. Please read the materials from the company regarding important points and potential side-effects before receiving your vaccination.
We also recommend that you consult your physician in advance if you have underlying conditions such as allergies or ongoing medical treatment, or other health concerns.
6. Starting dates for reservations and vaccinations
The next reservation period will be as follows:
・September 27 (Mon) starting from 9:00am
※All unvaccinated residents are eligible.
(Vaccinations for this reservation period will be administered between October 4 and October 24)
※Takikawa City has sent vaccination tickets to all eligible residents. Depending on the status of reservations during this period, as well as the status of influenza vaccinations, the city may consider reducing the number of healthcare facilities and/or limiting the days available for vaccinations during future reservation periods.
(※Even if you have a preferred healthcare facility in mind, please be aware that they may no longer be accepting reservations after this period.)
We strongly encourage anyone who is considering getting vaccinated to apply during this reservation period.

For 12-15 year-olds:
「-」= Not administering vaccinations (cannot take reservations).
「〇」= Must be accompanied by a parent or guardian.
「※1」= Must be accompanied by a parent or guardian in principle; however, if a parent or guardian reads the informational paper and signs the medical questionnaire, and cannot accompany their child for unavoidable reasons, then vaccinations can be administered for children in junior high school without a parent or guardian present.
「※2」= In addition to the conditions listed in「※1」, a parent or guardian must be present if the child has a history of severe allergies.
1) Regarding priority order
All residents at least 12 years old are eligible for vaccinations.

3. People under age 60 with underlying condition and have registered their status with the city ⇒ Vaccinations have begun

■Phone reservation system for pregnant women (Ninpu-anshin-yoyaku):
To protect the health of our mothers and children, Takikawa City will be implementing this system to ensure that pregnant women and their spouses who wish to be vaccinated can do so. The city will make calls to residents who have submitted a notification of pregnancy (ninshin-todoke) and not yet been vaccinated to help them secure appointments. (Phone calls will be conducted in Japanese)
※Due to limitations of vaccine supply from the national government and appointment availability, vaccinations for pregnant women themselves will be prioritized. For spouse-only appointments, please reserve online or with the Call Center.
2) Vaccination methods
In Takikawa, vaccinations are being administered in individual setting at healthcare facilities.
3) Vaccination tickets
■Takikawa City will send vaccination tickets to residents as they become eligible.
■Vaccination tickets were sent to residents aged 65 years and older on April 21, 2021, to those aged 60-64 years old on July 2, to those with underlying medical conditions who have registered their status with the city starting July 15, and to residents aged 50 years and older on July 26. Tickets for all other residents were sent out on August 16. If you do not receive your tickets by August 23, please contact the vaccination HQ.
■Children turning 12 years old after August 16 will have their tickets sent as their birthday approaches.
■The envelope with your tickets also contains medical questionnaires, a list of vaccination sites, and information on how to make a reservation.

5) On the day of your appointment
■You will receive your vaccination at the designated date and time at the designated site.
■Please bring your vaccine ticket, completed medical questionnaire, and personal ID (e.g. My Number card, driver’s license, health insurance card, residence card).
■You will be vaccinated after an interview with the doctor to confirm eligibility. You will be monitored for about 15~30 minutes following your vaccination.
※Reservations are necessary for effective administration of vaccines and avoiding the 3 Cs. Please only go to your designated healthcare facility on the date and time of your reserved appointment.

Establishment of Novel Coronavirus Vaccination HQ
In order to provide vaccines against the novel coronavirus to all residents without delay, a Novel Coronavirus Vaccination HQ was established by the Community Health Promotion Department to create and maintain the city’s vaccination system.
Currently, the HQ is making preparations to administer vaccinations against the novel coronavirus.

1) What are vaccines?
■Vaccines grant and strengthen immunity from infectious diseases.
■Patients can expect effects including: preventing the onset of an infectious disease; and preventing serious symptoms even if symptoms do present.
